Dozer Transport Bed Mount Failure 2016

On Tuesday, March 14, the Forest Safety Officer received a call from an Assistant Fire Management Officer informing that equipment operators had noticed the beds that are mounted onto truck transports were becoming detached. This detachment included missing and shredded mounting bolts and broken welds, both used to mount the beds to the truck frames. Equipment operators had noticed this while doing equipment inspections in preparation for that day’s operations. When this condition was discovered on one transport, the unit’s two other transports were also inspected. These inspections discovered similar indications of mounting failures— missing mounting bolts and broken welds.
