The engine, an open-backed, two-axle truck, was responding with red lights and siren on Etiwanda Avenue when it skidded and struck the train. The rear of the engine broadsided a freight car, throwing Krejci from the back of the vehicle, killing him. The other occupants of the engine were injured.
